kdev: batch git subprocesses — one ls-files and one blame per file
Previously CreateExtractor ran git ls-files per file, and Extract ran
git blame -L per matching line. Now ls-files runs once at factory
construction to build a tracked-file set, and blame runs once per file
at extractor creation with all line timestamps cached.
